What is a digital twin?

Imagine having a digital mechanistic model-based replica of a patient, enabling you to simulate various treatments and predict their outcomes – your “digital twin.” This powerful concept allows for personalized medicine by mirroring the patient's unique biology and characteristics, providing insights that can optimize treatment strategies and enhance healthcare decision-making.

The quantitative systems pharmacology digital twin approach outlined in this manuscript uses this concept to overcome the challenges of Phase 1 clinical trials, specifically to assess exposure-response relationships in a heterogeneous patient population with different clinical indications for mosunetuzumab, a T-cell engaging bispecific antibody. We demonstrate a richer, more comprehensive understanding of its therapeutic effect that complements other empirical methods utilized to inform clinical dose-response relationships. Furthermore, by leveraging simulations of digital twins, we were able to evaluate potential baseline tumor characteristics as predictive biomarkers of patient response.
